When you bring your completed application, please bring a current DMV printout, as well as a Social Security printout.  Your Social Security printout should show your prior work/wage history. 

You can request this documentation at your local DMV office and your local Social Security Administration office. 

In sum, be sure to bring the three following documents with you:

1. Completed Employment Application
2. Current DMV printout from your local DMV office
3. Current Social Security printout from your local Social Security Administration office
